DC's medical cannabis program is run by the Alcoholic Beverage and Cannabis Administration (ABCA). Since August 18, 2026, new registrations go through ABCA's newer MyABCA customer portal. People with a valid registration from the old system keep using it until it is time to renew.

Registration is free and lasts two years. Adults 21 and over can self-certify, and people 18 to 20 need a recommendation number from a healthcare practitioner. You will need a DC REAL ID, or a government photo ID plus proof of DC residency such as a utility bill or lease. A temporary digital registration valid for 90 days is issued when you submit, so you can shop straight away.
Domestic and international visitors 21 and over can register as temporary patients. You pick a validity period from 30 days to a year, and ABCA lists the fee at $10 to $100 depending on that period. Non-resident registrations are digital only.
If you already have a medical cannabis card from a jurisdiction DC recognizes, you may not need a DC registration at all. Check the ABCA reciprocity list and bring your card or digital credential with photo ID.
